  • Online forms accessible with a click. Accessible anytime, you can grab forms including life and health forms and notices, benefit booklets, and benefit summaries on our website.
  • High-quality Georgia-based in-network providers and choice of out-of-network providers. You have access to the best doctors and healthcare facilities for all of your needs. You can review a list of these providers here. Depending on your plan, you still have a choice of out-of-network providers.

Am I Eligible?

You are eligible for the GMEBS Life and Health Insurance Fund if you are a:
 
  • Municipality
  • Municipal authority
  • Municipal commission
  • Housing authority
  • Regional commission
In general, to be eligible, an employee must work in a position that requires 30 or more hours of service per week and is expected to last 48 weeks. Certain large employers may also offer coverage to other workers who meet the definition of a “full-time employee” under the Affordable Care Act. City employers also may elect to offer coverage to elected and appointed members of their governing body, chief and associate legal officers, and municipal judges.
We offer five different health insurance plans to meet your needs. Each plan differs in deductibles, co-insurance, copays, and provider networks.
 
Preferred provider organization (PPO) 90/70
  • Pays 90% of eligible in-network claims
  • Pays 70% of out-of-network claims
  • Your choice of a $500, $750, $1000, or $1500 deductible
Preferred provider organization (PPO) 80/60
  • Pays 80% of eligible in-network claims
  • Pays 60% of out-of-network claims
  • Your choice of a $500, $750, $1000, or $1500 deductible
Point of service (POS) 80/60
  • Pays 80% of eligible in-network claims
  • Pays 60% of out-of-network claims
  • Your choice of a $500, $750, $1000, or $1500 deductible
Health Maintenance Organization (HMO) 90
  • Pays 90% of eligible in-network claims
  • No coverage for out-of-network claims
  • No required deductible
Health Maintenance Organization (HMO) 80
  • Pays 80% of eligible in-network claims
  • No coverage for out-of-network claims
  • Individual deductible of $150
  • Family deductible of $450
Dental Insurance
  • 100% coverage for preventative treatment and diagnostic exams
  • 80% for basic prosthetic treatment
  • 50% for major and restorative work
  • 50% up to a lifetime maximum of $1,000 for orthodontia
Short-Term Disability Coverage
  • Provides a weekly benefit amount to your employees who become totally disabled due to an illness or accidental injury.
  • Members choose the amount of coverage to provide for employees.
Life Insurance Options
In addition to health insurance coverage, the GMEBS Life and Health Insurance Fund offers you the following life insurance coverage:
 
  • Group Term Life Insurance. You have the flexibility to choose the amount of life insurance coverage offered to employees and dependents.
  • Optional Life InsuranceAllow employees to purchase life insurance in addition to the amount you provide. In order to add this benefit to your city, 30% of employees must elect to participate.
  • Accidental Death and Dismemberment. Covers death or specific types of injury as a result of an accident.
  • Retiree Life Insurance. As a fund member, you can participate in a program offering term life insurance benefits for retirees. The benefit must be 100% employer paid and all premium charges will be billed on a monthly basis by GMA to the employer.
